Referral partners

Help a participant take their next step.

Start with consent and a few basic details. We’ll discuss the participant’s goals, service fit, availability and funding with you, keeping their choices at the centre.

Working alongside you

We welcome referrals from support coordinators, plan managers, LACs, schools, TAFEs and health services.

We also work with Aboriginal community controlled organisations, multicultural and settlement services, and community justice and legal services.

What helps us get started

  • Your contact details, role and organisation.
  • The participant’s first name and general support interests.
  • Location and preferred communication method.
  • Confirmation that you have consent to share these details.

Please do not send medical records, NDIS plan documents or sensitive histories through the public form. Detailed information can be discussed through an agreed process.

Service suitability, availability and funding are confirmed individually.

What would you like support to make possible?

Tell us what matters to you. We’ll discuss your needs, suitable supports and funding before you decide. Sending an enquiry does not commit you to services.
