EMPOWERPATH AUSTRALIA

Privacy & confidentiality

Know what you share and how it is used. This page explains website enquiries and referrals, your privacy choices and how to ask for help. Sending an enquiry does not sign you up for support.

What information we collect

When you send an enquiry, we collect your name, phone number, email, who the enquiry is for, the support you are looking for, your preferred contact method and your message. You can also choose to tell us about language or accessibility needs. We record your agreement to be contacted.

When someone makes a referral

A professional referral also asks for the referrer’s organisation and role, and the participant’s first name and suburb or region. Only share someone else’s details with their permission. Explain what you are sharing and why, in a way they can understand.

Why we ask for these details

We use this information to respond to your enquiry or referral, understand the support you are looking for and discuss possible next steps. Sending a form does not mean you have agreed to begin services.

Share only what is needed

Please do not send medical records, NDIS plan documents or other sensitive records through the website form. A short description of your goals and support needs is enough to start a conversation. If more information is needed, ask our team how to share it safely.

Keeping your information private

Website enquiries and referrals are stored privately, not published on the website or made available to other visitors. We treat your information as confidential and only share it for an agreed purpose or when required by law. If you want a family member, advocate or another person involved, tell us who they are and what you agree we can share.

Your choices and questions

You can ask what information we hold about you, request access or a correction, or tell us if your contact preferences have changed. Email info@empowerpathau.com. To protect your privacy, we may need to check your identity before providing personal information. You can ask a trusted person to help you contact us.

How long information is kept

This page covers website enquiries and referrals, not a complete service privacy policy. Detailed arrangements for service records, including how long they are kept, will be explained during onboarding. You can ask about retention or request deletion of your website message; some information may need to be kept to meet legal obligations.

If you have a privacy concern

Contact info@empowerpathau.com and tell us what concerns you and how you would like us to respond. You do not need to send sensitive documents to raise a concern. You can also seek information or raise a privacy complaint with the Office of the Australian Information Commissioner (OAIC).

Information in a way that works for you

Tell us if you need an interpreter, plain English information or another way to communicate. We will discuss your needs and available arrangements with you.
